Registration Requirements USYSA:
Following are the requirements the IYSA imposes on teams playing in IYSA Sanctioned events, and teams that travel
as a USYS team:
1.) A copy of your State Association approved roster.
2.) One completed copies of IYSA Tournament Roster
including guest players.
a.) U8 and U9 teams are allowed 12 players on the roster.
b.) U10 through U12 teams are allowed 14 players on the roster.
b.) U13 through U17 teams are allowed 18 players on the roster.
b.) U18 through U19 teams are allowed 22 players on the roster.
(can only dress 18 players per game).
3.) Illinois Medical Release only. Per the IYSA, we must have signed IYSA Medical Release only, we keep a copy
of each and they need not be notorized.
4.) One copies of Guest Player Forms approved by your state association.
5.) Travel Permit from your state association if traveling from outside of Illinois.
6.) Approved and laminated player passes.
7.) If you are traveling as a US CLUB SOCCER team, you will need everything on Form NT.
All copies of registration forms will be retained by Moline United.
